It shoul say at the top of your check. Theres a lil piece at the top that you tear off and it should say on that

I do not know were to go to ask this, Maybe you can help me. I get workman crop check each week, A couple of months ago, I got release from the employer Doctor, with a restriction to my left shoulder, with no lefting no more than 5 lbs. My boss said that I can not reture to work, until I am 100%. I work for a disabliting business, The place is called the CDC RESOURECE in Indiana. I like to know, if I can get unemployment well getting workman crop .Iam asking cause workman crop dose not come out of my employment pocket. My shoulder dose not stop me from doing my job. PLEASE HELP ME .If you have something to ask me ,I can answer them. THANK-YOU KIMBERLY VESSELS
